SUMMARY: The Lead Family Specialist is a supervisory level position responsible for the leadership, supervision, and direct oversight of the DULCE Family Specialist personnel, 2 staff members. Family Specialist role is to provide health and wellness support as well as education to families in the pediatric care setting. Vital to the role is responsibility over data collection, performing patient documentation in the EHR, and participating in quality improvement activities, training, and national meetings as needed. This is a family facing position and reports to the Program Manager.

Full-time exempt position, annual wage range 45800 to 47000 depending on experience.

Position will work out of the Tampa Family Health Center on Osborne Ave

ESSENTIAL RESPONSIBILITIES, DUTIES, AND TASKS:

  1. Supervise, support, and train assigned staff including hiring staff, providing individual supervision, conducting regular staff meetings, and completing performance evaluations.
  2. Responsible for a partial case load.
  3. Responsible for establishing trusting relationships with patients and their families.
  4. Meet with families at routine Well Child Visits and telephone check-ins, supporting the parent-child relationship, providing information on child development, and supporting unmet social needs by coordinating referrals for concrete supports.
  5. Provide ongoing parent/newborn support through phone, test, email, and community visits with a focus on parental and cultural strengths.
  6. Continually assess the need for additional resources for the family, especially relating to developmental concerns by building strong, honest, trusting partnerships with families that can address concerns in a supportive manner.
  7. Collaborate with the legal partners to ensure families are provided concrete supports.
  8. Administer screening tools to ensure that families are supported in caring for their infants and adhering to the recommended plan of care.
  9. Always maintain HIPAA and confidentiality compliance, strictly adhere to policies which ensure participant confidentiality.
  10. Exhibit excellent working relationships with patients, visitors, colleagues, and facility personnel.
  11. Engage in individual reflective supervision sessions.
  12. Timely data entry into participant records and data collection systems.
  13. Prepare for and participate in weekly case reviews with the Interdisciplinary Team.
  14. Participate in quality improvement activities.
  15. Participate in outreach and community events.
  16. Participate in related training and workshops.
  17. Perform other related duties as assigned or requested.
  18. Strictly adhere to policies which ensure participant confidentiality.

REQUIREMENTS:

  1. Bachelor’s degree in social work, public health, child development or other related fields. May consider associate degree and relevant equivalent job experience.
  2. Minimum of 3 years working with young children (especially newborns) and their families in community-based setting. Some supervisory experience.
  3. Eligibility or certification as a Community Health Worker (CHW)
  4. Working knowledge of Microsoft 365 applications such as Outlook, Word, Excel and SharePoint.
  5. Commitment to improving infant and family health through innovative approaches that address the SDOH/HRSN.
  6. Ability to work with many diverse people, to understand the community served and community connectiveness.
  7. High energy, positive, “can do" attitude, flexibility, teamwork, and attention to detail: high degree of initiative.
  8. Must be punctual for scheduled work and use time appropriately. In-person position.
  9. Bilingual, English/Spanish, preferred but not required.
  10. Ability to walk, stand, climb stairs, kneel, bend, reach, and manipulate objects; move materials up to 10 pounds on a regular basis; infrequently require moving materials weighing up to 20 pounds.
  11. Must be sensitive to the cultural and socioeconomic characteristics of the population that Champions for Children serves.
  12. Successfully complete a Level 2 criminal background check, motor vehicle records check and drug screening.
