Healthy Families Family Support Specialist provides support and assistance to optimize the home environment for the physical, emotional, and social well-being of the parent/child family units assigned. Visits with participants are in their home. Works under the family support specialist supervisor. Participates in the organization’s performance and quality improvement efforts.

40 hours per week, full-time regular position. Non-exempt, $18.00 per hour.

Position is based at Champions for Children office located at 12937 Telecom Parkway, Tampa, FL 33637

ESSENTIAL FUNCTIONS:

  • Establishes a trusting relationship with families assigned to caseload by initiating and maintaining regular client contact. Incumbent will strive to engage assigned families into the Program.
  • Complete home visits in the frequency as required by individual levels of service.
  • Encourages compliance with, monitors and documents child immunization schedules measured at 6, 12, 18 & 24 months, and well-baby check- ups measured at 6, 12, 18, 24, months as well as age 3- 5.
  • Links participating family members to a medical provider whenever necessary and appropriate.
  • FSW identifies and assists families in accessing needed resources and social services. Documents referrals and outcomes of those referrals for the tracking system.
  • Works with assigned families to develop, monitor and implement a Family Support Plan (FSP). Observes and meets relevant service requirements and guidelines.
  • Administers developmental assessment tools at established intervals, and other established assessments in a timely manner ( ie: ASQ-3, ASQ-SE, HFPI, Home Safety checklist, Edinburgh, etc.);
  • Promotes effective parent-child interaction through teachings and activities that enhance and strengthen bonding and parenting skills. Models appropriate activities during each home visit as measured by case progress notes. Provides in-home parent education to families, makes referrals to community parenting classes, as appropriate. Implements and promotes activities as per curricula used and recommended by Program (i.e.:GGK)
  • Record-keeping. Establishes an individual record for each family and maintains accurate documentation of any and all activities related to service delivery. Prepares Home Visit Notes and other documentation on a timely manner. Complies with Agency and Program requirements, policies and procedures relative to the proper establishment, handling and storing of client records. Understands and complies with confidentiality guidelines. Documents home visits, collateral contacts and all other activities on a daily basis. Complies with employer’s requirements regarding accuracy, completion and timeliness of documentation.
  • Meets with Supervisor weekly for review of case management and other relevant activities of all families in caseload. Is prepared for supervision prior to the appointed time. Achieves at least 80% supervision rate.
  • Manages time and resources effectively through good organization and prioritization of work and through use of positive problem solving skills.
  • Travels as required for hospital visits, home visits, educational programs or meetings; mileage is correctly documented, compiled and submitted monthly for reimbursement.
  • Attends training conferences/workshops, in-house trainings, staff meetings, and other staff development activities, as required and in a timely manner.
  • Collects data required for reports. Completes reports as required.
  • Complies with job preparedness requirements by participating in Agency / Program / Position - related trainings as required, within the time parameters established by policies & procedures.
  • Performs other related duties as assigned.

Qualifications and Requirements:

  • Minimum: a H.S. diploma or GED
  • 2-3 years’ experience working with children and families
  • Healthy Families Family Support Specialist training will be provided
  • Excellent written and verbal communication skills, English and Spanish preferred
  • Bilingual English/Spanish is required.
  • Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense of license suspension.
  • Must possess means of providing job related transportation and show proof of required insurance. Must be insurable under CFC’s current liability insurance policy.
  • Must be able to work flexible hours to accommodate participant's schedules.
  • Must successfully complete and maintain FDLE clearance, Federal background and state criminal background check, sexual predator screening and drug screening.
  • Must be sensitive to the socioeconomically and culturally diverse population Champions for Children serves.
  • Must have a reliable car, active auto insurance, and valid Florida driver’s license
