As a Life Skills Coach, you will be providing person-centered direct support for a 61-year-old lady who is a recent amputee, at times uses a non-self-propelled wheelchair, and is new to the Trooper, PA area. She desires to re-learn independent living skills on a one-to-one basis while in her home and throughout her community.

Experienced staff will:
  • Be responsible for supervision, health and safety, and personal care support, of individual at all times per ISP
  • Be mindful of supporting an Everyday Life, by consistently reinforcing the individuals Individual Support Plan (ISP) outcomes.
  • Provide skills training and assistance while engaging with individual in the areas of daily living, personal care, community, and social integration.
  • Have basic knowledge of diabetes and willing to encourage and assist to follow diabetic care protocol for the safety of the individual
  • Perform all other duties as assigned.
Job Responsibilities
  • Provide training, guidance, and support to help the individual develop and improve skills necessary for independence, such as activities of daily living (learning to cook for a healthy and satisfying life style and take care of new home), mobility (ambulation and community accessibility), communication (ex. Appointment/Scheduling), and self-care (body, mind, and self esteem)
Offer appropriate levels of assistance and supervision based on the individual's needs and abilities, ensuring they have the support they require while encouraging independence.
Encourage and motivate the individual to take initiative, try new tasks, and overcome challenges as they work towards greater independence.
  • Assist to identify barriers to independence and assist to create and implement adaptive strategies or modifications to the environment, tasks, or routines to promote greater autonomy.
Assist the individual in problem-solving and finding solutions to obstacles or difficulties they may encounter in their pursuit of independence.
Foster a sense of empowerment and self-confidence in the individual, emphasizing their strengths and abilities rather than focusing solely on limitations.
  • Communicate information consistently by maintaining accurate and thorough documentation of the individual's progress, goals, interventions, and report any changes in their needs, conditions, or abilities to Support Team
  • Remind individual to take blood sugar as prescribed and document date, time, and result.
  • Present with a clean and neat appearance.
  • Respect and Strive to Enrich the Life of the Individual
Job Qualifications
  • Required current Certified Nursing Assistant (CNA) certification in the state of Pennsylvania or Bachelors Degree in Psychology, Education, Spec. Education, and Social Work.
  • Have at least 6 months experience with amputee care.
  • Have at least 6 months experience working with diabetes and food regimen.
  • Have a sincere desire to work with individual and demonstrate sensitivity to the needs.
  • Kind, respectful, patient and cheerful when engaging with individual, co-workers, family/support team, and community.
