ABOUT THIS ROLE

As our full-time Intensive Care Coordinator at Hoyleton Youth and Family Services, you evaluate new referrals, contact families promptly, and conduct comprehensive assessments to create personalized service plans. Your day involves updating plans, developing Crisis Prevention and Safety plans, and facilitating Child and Family team meetings. You engage with families through weekly check-ins and monthly home visits, collaborate with treatment providers and schools, and ensure meticulous case documentation. Managing a caseload of up to 25 families, you deliver strength-based, family-centered, culturally competent, and trauma-informed services, making a tangible difference every day.


REQUIREMENTS

  • Bachelor's degree in counseling and guidance, rehabilitation, social work, education, psychology, or related human service field, along with two years of supervised experience
  • Ability and willingness to actively participate in a fast-paced, multidisciplinary team environment
  • Knowledge of community programs, services, evidence-based practices, and a variety of treatment modalities
  • Possess and maintain a valid driver's license and provide proof of insurance
  • Strong organization and time management skills
  • Excellent interpersonal and written communication skills
  • Must possess the knowledge, personal qualities, and practice skills to work effectively with professionals in other organizations and from other disciplines
