Salary Range: $34.00 - $39.00 Based on experience level

Key Responsibilities:

  • Patient Experience Focus: Prioritize the well-being and satisfaction of patients in all your actions.
  • Acute Patient Triage: Efficiently assess and triage acute patient complaints.
  • Medical Assessments: Provide accurate, detailed assessments to ensure safe and effective care.
  • Resource Support: Help patients access health resources and social services.
  • Performance and Quality Goals: Support the primary care team in achieving performance measures and quality improvement objectives.
  • Collaborative Care: Work closely with the Care Team to coordinate and manage patient care.
  • Safety and Quality: Deliver high-quality, safe care in a culture that prioritizes patient safety.
  • Health Screening: Perform health screenings and lab procedures under provider direction.
  • Patient Chart Preparation: Assist with patient intake and discharge procedures.
  • Care Management: Collaborate with leadership to develop and maintain care management systems for high-risk patients.
  • Patient Engagement: Encourage patients in their self-management and provide education to improve health outcomes.
  • Care Coordination: Ensure seamless, coordinated care to prevent treatment gaps, especially during care transitions.
  • Teamwork and Collaboration: Foster a team-oriented environment with providers and care team members.

Qualifications:

  • Current New Hampshire Nursing License
  • Experience in primary care and ambulatory health care preferred
  • Current BLS certification

This position is eligible for a one-time sign-on bonus of up to $5,000.00.

Our comprehensive benefits package includes medical and dental insurance with an agency subsidy. One of the GSCH health insurance plans has a no-premium option for employee-only coverage. GSCH offers a 401(k) plan with an employer match, employer-paid group term life insurance, and short-term disability coverage. Voluntary benefits include vision, long-term disability coverage, Flexible Spending Account, and access to local discounts. GSCH offers earned time accrual (15 days in your first year [pro-rated by FTE]), plus seven (7) paid holidays. Eligibility starts at 24 hours/week, with full benefits offered at 32 hours/week and above.
