Our mission is to enhance health outcomes through effective communication, care coordination, and community engagement. As a Float Nurse, you will be instrumental in advocating for patients, ensuring they receive the necessary resources and support throughout their healthcare journey.
Key Responsibilities:
- Deliver patient-centered care that promotes health and wellness.
- Utilize clinical judgment and critical thinking to assess patient conditions and develop appropriate care plans.
- Engage in effective communication with team members to ensure seamless care delivery.
- Participate in quality improvement initiatives to enhance patient care services.
- Support patients in managing chronic conditions and navigating health transitions.
- Active Nursing License in New Hampshire.
- Experience in primary care or ambulatory settings is preferred.
- Current Basic Life Support (BLS) certification.
- Willingness to travel between multiple locations as needed.
- Strong interpersonal skills and the ability to work effectively with diverse populations.
- Excellent organizational and clinical assessment abilities.
- Compassionate demeanor and a commitment to patient advocacy.
Full-Time, 40 hours per week, Monday to Friday.
