Our mission is to enhance health outcomes through effective communication, care coordination, and community engagement. As a Float Nurse, you will be instrumental in advocating for patients, ensuring they receive the necessary resources and support throughout their healthcare journey.

Key Responsibilities:
  • Deliver patient-centered care that promotes health and wellness.
  • Utilize clinical judgment and critical thinking to assess patient conditions and develop appropriate care plans.
  • Engage in effective communication with team members to ensure seamless care delivery.
  • Participate in quality improvement initiatives to enhance patient care services.
  • Support patients in managing chronic conditions and navigating health transitions.
Qualifications:
  • Active Nursing License in New Hampshire.
  • Experience in primary care or ambulatory settings is preferred.
  • Current Basic Life Support (BLS) certification.
  • Willingness to travel between multiple locations as needed.

Work Schedule:
