What You'll Do As a Hospice Social Worker, you will:
- Conduct comprehensive psychosocial assessments of hospice patients and their families.
- Provide individual and family counseling, addressing grief, loss, and end-of-life decision-making.
- Facilitate advance care planning and decision support.
- Connect patients and families with community resources and support services.
- Provide bereavement support and follow-up with families after the loss of a loved one.
As a Bereavement Coordinator, you will:
- Conduct assessments of bereaved family members and caregivers.
- Provide individual and group counseling through the grieving process.
- Facilitate grief support groups and memorial services.
- Engage in community outreach and provide education on the grieving process and bereavement support strategies.
