Our approach includes a unique pre-hospice care model, a proprietary patient identification algorithm, and a partnership mindset with provider groups. We are committed to offering the most comprehensive end-of-life care solutions through best-in-class hospice operations.

What You'll Do As a Hospice Social Worker, you will:

  • Conduct comprehensive psychosocial assessments of hospice patients and their families.
  • Provide individual and family counseling, addressing grief, loss, and end-of-life decision-making.
  • Facilitate advance care planning and decision support.
  • Connect patients and families with community resources and support services.
  • Provide bereavement support and follow-up with families after the loss of a loved one.

As a Bereavement Coordinator, you will:

  • Conduct assessments of bereaved family members and caregivers.
  • Provide individual and group counseling through the grieving process.
  • Facilitate grief support groups and memorial services.
  • Engage in community outreach and provide education on the grieving process and bereavement support strategies.
