Role of the Licensed Practical Nurse (LPN)

As an LPN, you will play a crucial role in collaborating with our interdisciplinary clinical teams, including physicians, social workers, chaplains, and therapists, to fulfill our mission. Your ability to maintain effective communication with patients, staff, and management will be essential in providing exceptional care.

Key Responsibilities
  • Assist the RN in conducting initial and ongoing assessments of patients' needs related to their terminal diagnosis, focusing on physical, functional, psycho-social, and environmental aspects.
  • Collaborate with the healthcare team to develop and manage individualized care plans, prioritizing services based on patient and family requirements, and suggesting modifications as needed.
  • Educate patients and families about the disease process, self-care strategies, end-of-life considerations, and ethical issues.
  • Communicate effectively with the RN, medical director, and other healthcare professionals regarding any changes in patient conditions and the outcomes of interventions.
  • Document observations, findings, and services rendered accurately and timely.
  • Perform additional related duties as required.
