Overview of the Position:

Under the direct supervision of the Client Manager, the Specialized Medical Case Manager (SMCM) will provide a strengths-based, re-engagement individual level intervention that is focused on re-engaging HIV positive clients who are at risk for falling out of care or who have fallen out of care.

The SMCM works with clients to identify and utilize the client’s current personal strengths to address and overcome barriers preventing the client from continuously engaging in care and treatment. By allowing clients to prioritize their own service needs, the SMCM empowers clients to take charge of their care and build a plan that feels best suited to his/her individual needs. Additionally, the SMCM seeks to engage and expand client support systems whenever possible. In order to provide more intensive on-on-one services, the SMCM maintains a smaller caseload than traditional medical case managers. The SMCM’s caseload, however, fluctuates as clients return to care and others are successfully re-engaged and transitioned to less intensive, traditional medical case management.

Education and Experience:

An HIV Medical Case Manager must meet one (1) of the following qualifications:

Qualifications:

1. Hold a master’s degree from an accredited college or university in a human services field including, but not limited to, Social Work, Sociology, Health Education, Child and Family Development, Counseling, Psychology, or Nursing; or

2. Hold a bachelor’s degree from an accredited school of social work; or

3. Hold a four-year degree form an accredited college or university in a human services field or related curriculum including at least fifteen (15) semester hours in courses related to social work or counseling and six (6) months of social work or counseling experience; or

4. Hold a four-year degree from an accredited college or university and one (1) year experience in counseling or in a related human services field which provides experience in techniques of counseling, casework, health education, group work or community organization.

Essential Job Functions:

The SMCM provides a range of services that link clients with medical care, psychosocial services, benefits and other supportive services through:

a) Assists the Outreach Specialist with contacts to clients via telephone and conducting research to find updated contact information;

b) Works closely with Outreach Specialists as clients are located and prepared to re-engage agency services and medical care;

c) Interprets clients responses on barriers to care and treatment using statewide tool Return to Care Assessment (RTC)

d) Customizes services through the development of an Enhanced Care Plan (ECP) with client to re-align the client’s perception/prior experience of service usefulness and accessibility barriers, allowing the client to set service priority

e) Communicates with management on necessary service enhancements

f) Emphasizes family/partner/other support systems

g) Provides goal-oriented, time-specific Specialized Medical Case Management activities and support to encourage the clients’ participation in his/her own care plan.

h) Works proactively with clients until they are ready for less intensive, traditional case management (i.e. SMCM must often take the initiative and follow the client’s medical appointment and other service referrals very carefully)

i) Identifies emerging barriers that may prevent each client from maintaining participation in care and treatment

j) Coordinates and links clients to services required to implement the ECP

k) Monitors to assess the efficacy of the ECP, to include home/field visits as necessary

l) Reviews and re-evaluates the ECP on a frequent basis, at a bi-weekly minimum Enrolls and recertifies clients in SC ADAP (AIDS Drugs Assistance Program) as needed

Continual CQI

• Provides benefits counseling, support and information, including risk reduction and service literacy expansion

• Provides advocacy, referrals to and coordination with, public or private programs, including hospitals, physicians, clergy, and community agencies to ensure continuity of care in the home or an institutional setting

• Provides accurate and complete documentation of activity through Progress Logs, Services Provided, Enhanced Care Plan, transitional communication in format SBAR (Situation, Background, Assessment, Recommendation), and other project-specific data and evaluation collection methods in Provide Enterprise (PE)

• Maintains knowledge of community resources and methods to access services

• Utilizes evidence-based, interviewing techniques, including but not limited to Motivational Interviewing, Trauma Informed Care, Basic Counseling, Health Literacy Expansion, etc.

• Attends necessary trainings/meetings, such as monthly outreach meetings at SC Department of Health and Environmental Control (SC DHEC) in Columbia, SC

• Completes Medical Case Management required trainings within 18 months of hire

• Assists clients in obtaining housing services on an as needed basis

• Takes time to explain the reasoning associated with program requirements, which will allow the client to better understand service requirements

Physical Requirements:

Must be able to lift 30 pounds. Vision and hearing corrected to within normal limits is required. Must have manual dexterity to key in data; utilize computer, grab, grip, hold, tear, cut, sort, and reach.
