DUTIES:
- Ensure the active treatment is occurring for all residents and is consistently implemented.
- Act as the liaison between all outside entities, including family, guardians, community support programs, and others, to coordinate treatment related objectives.
- Monitor progress with individual program plans and assure changes are initiated as necessary.
- Provide recommendations to other team members related to treatment goals and objectives.
- Ensure the adequate environmental supports and assistive devices are present to promote independence.
QUALIFICATIONS:
Bachelor's Degree in Human Services field required. At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
