Job Title: Case Manager
Job Summary:
Spurwink Services is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and monitoring the treatment programs for individuals with intellectual disabilities. This role requires strong communication and coordination skills, as well as the ability to work collaboratively with various departments and outside entities.
Key Responsibilities:
- Develop and implement individualized treatment plans to meet the unique needs of each client.
- Coordinate with family members, guardians, and community support programs to ensure seamless care and services.
- Monitor progress and make recommendations to ensure the active treatment is occurring and is consistently implemented.
- Provide recommendations to team members related to treatment goals and objectives.
- Ensure the adequate environmental supports and assistive devices are present to promote independence.
Requirements:
- Bachelor's Degree in a Human Services field.
- At least one year of experience working directly with individuals with intellectual disabilities or other developmental disabilities.
