Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Spurwink Services. As a Case Manager, you will play a critical role in coordinating and monitoring the treatment programs for individuals with intellectual disabilities.

Key Responsibilities
  • Develop and implement individualized treatment plans to ensure the active treatment is occurring for all residents.
  • Act as a liaison between outside entities, including family, guardians, and community support programs, to coordinate treatment-related objectives.
  • Monitor progress with individual program plans and initiate changes as necessary.
  • Provide recommendations to team members related to treatment goals and objectives.
  • Ensure adequate environmental supports and assistive devices are present to promote independence.
Requirements

To be successful in this role, you will need a Bachelor's Degree in a Human Services field and at least one year of experience working directly with individuals with intellectual disabilities or other developmental disabilities.
