Job Duration: 13 weeks
Orientation: All Hours Billable

Responsibilities:

  • Provide comprehensive case management and social work services to patients.
  • Conduct discharge planning and coordinate with healthcare providers to ensure smooth transitions.
  • Utilize Allscripts, CarePort, and Wellsky systems for documentation and case management.
  • Assess patient needs and develop individualized care plans.
  • Collaborate with medical staff, patients, and families to ensure the best possible care.
  • Maintain accurate and detailed patient records.
  • Adhere to facility guidelines and protocols, including COVID-19 and flu vaccination requirements.

Qualifications:

  • Master’s degree in Social Work (MSW).
  • Minimum of 2 years of experience in social work and discharge planning.
  • Current certifications in ACLS and BLS.
  • Experience with Allscripts/CarePort/Wellsky preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • No first-time travelers accepted.
  • Locals accepted.

Certifications Required:

  • ACLS (Advanced Cardiovascular Life Support)
  • BLS (Basic Life Support)

COVID-19 Vaccine (Facility Guideline):

  • Required, with Medical Exemptions Only

Flu Vaccine (Facility Guideline):

  • Required, with Medical/Religious Exemptions and Declinations Allowed
