Home Health Clinical Review Nurse Coordinator

An outstanding hospital in Southwest Colorado is in search of a new Home Health Clinical Review Nurse Coordinator.
This hospital has earned numerous awards for units throughout their facility. In addition, this 50+ bed hospital is accredited in many areas, but notably has a glowing record in their Home Health Services. The department is a part of a fantastic system that has received performance achievement awards as well as other accolades. The Home Health Clinical Review Nurse Coordinator will join an organization with an outstanding reputation in the community and across the Central US.
The Home Health Clinical Review Nurse Coordinator will report directly to the Home Health Manager. Supported by a team of home health nurses, case workers, and clinical review committee this person will join a fantastic team of dedicated nurses willing to help them learn and grow. The ideal candidate will have a background in Home Health or Hospice care.
The Home Health Clinical Review Nurse Coordinator will lead and review the appropriateness of clinical care and documentation practices related to patients/residents. The Home Health Clinical Review Nurse Coordinator must assist the manager with training and coordinating clinical review and quality assurance as well. The Home Health Clinical Review Nurse Coordinator must be able to provide Chart audits, oversight and performance improvement planning for all programs. The position also requires collaboration with team members, other RNs, and Physicians to be able to provide the best care and outcomes possible. Critical thinking skills are a necessity. The Home Health Clinical Review Nurse Coordinator will also Assist with EMR best practices and policy review and maintenance.
