Title:Care Manager - Care Management Location:MOB 4th Floor Position Shift:PRN Position Summary:The purpose of the Case Manager position is to work within a defined patient population to promote the achievement of optimal clinical and resource outcomes
CM is responsible for facilitating appropriate lengths of stay and reimbursement for all hospital admissions in accordance with its goals and objectives
CM acts as the key information and education resource for the interdisciplinary team, including physicians
CM analyzes current systems and variances to identify opportunities for improvement
CM works to promote quality of care through collaboration with all service team members, patients and families
CM works with the leadership team to align the goals and visions
