The Nurse Care Manager plays a crucial role within the integrated care team, delivering personalized and population-focused health interventions to Medicare beneficiaries. This position is dedicated to managing patients with high-risk chronic conditions, ensuring effective education, self-management support, and timely healthcare delivery to achieve optimal health outcomes.

Key Responsibilities:

  • Oversee nurse care management for Medicare patients identified through collaboration with healthcare providers and practice staff.
  • Develop and implement a comprehensive care management plan annually, addressing the unique needs of each patient and their families.
  • Establish mutually agreed-upon care management plans with clear self-management goals and objectives.
  • Assess the effectiveness of care plans, making necessary adjustments to reflect changing patient needs.
  • Maintain precise and timely documentation in the electronic medical record (EMR).
  • Regularly review utilization and quality reports to identify patients requiring additional care management support.
  • Collaborate with pharmacies to educate primary care providers on prescription benefits and enhance medication adherence.
  • Monitor health information technology tools to track targeted populations and report on treatment and medication alerts.
  • Conduct medication reconciliations for Medicare patients during care transitions.
  • Provide education on medical conditions, treatment options, and strategies to prevent unnecessary emergency department visits.
  • Encourage patients to utilize their primary care provider for annual wellness visits and chronic condition management.
  • Work closely with the Community Health Worker team to ensure successful screenings related to social determinants of health.
  • Meet service provision expectations in alignment with client needs and organizational guidelines.
  • Collaborate as part of a multidisciplinary team, participating in program and integrated meetings as required.
  • Seek culturally competent services to address the diverse needs of patients and their families.
  • Perform all duties within the scope of practice for Licensed Practical Nurses as per state regulations.
  • Other responsibilities as assigned.

Working Conditions:

Regular exposure to blood, body fluids, and biohazardous materials is expected in this role.
