The Nurse Care Manager plays a crucial role within our integrated care team, focusing on delivering tailored health interventions to individuals enrolled in Medicare. This position is dedicated to managing patients with chronic illnesses and high-risk conditions, ensuring they receive the education and support necessary for effective self-management and timely healthcare access.

Key Responsibilities:

  • Oversee the care management of Medicare patients identified through collaboration with healthcare providers and practice staff.
  • Develop and implement personalized care management plans that address the unique needs of each patient, involving their families in the process.
  • Establish mutual care goals and objectives that are action-oriented and agreed upon by both the healthcare team and the patient.
  • Continuously evaluate the effectiveness of care plans, making adjustments as necessary to meet evolving patient needs.
  • Maintain precise and timely documentation within the electronic medical record (EMR).
  • Regularly review utilization and quality reports to identify patients requiring additional support.
  • Collaborate with pharmacies to enhance medication adherence and educate primary care providers on prescription benefits.
  • Monitor relevant health information technology tools to track patient progress and address any alerts or admissions.
  • Conduct medication reconciliations during care transitions to ensure continuity of care.
  • Provide educational resources to patients regarding their medical conditions, treatment options, and preventive measures.
  • Encourage patients to utilize their primary care provider for annual wellness visits and chronic condition management.
  • Work closely with the Community Health Worker team to address social determinants of health.
  • Meet service provision expectations in alignment with company guidelines.
  • Participate as a member of a multidisciplinary team, attending necessary meetings and collaborative sessions.
  • Seek culturally competent services to address the diverse needs of our patients and their families.
  • Perform all duties within the scope of practice as outlined for Licensed Practical Nurses.
  • Other responsibilities as assigned.

Working Conditions:

Regular exposure to blood, body fluids, and biohazardous materials is expected in this role.
