Description


The Community Health Educator will primarily work to plan, coordinate and implement community wellness education programs designed to promote wellness and prevent and/or manage disease. The Community Health Educator will also work with a specific target population in their homes to reinforce health habits and assist individuals in meeting their personal health-related goals while increasing self-management skills.

Essential Functions

  • Actively plans, organizes, and implements community wellness programs including but not limited to, community group presentations and evidence-based programs.
  • Actively participates in the development, promotion, implementation and evaluation of all community outreach education activities.
  • Accurately and consistently documents activities, recording information such as the numbers of presentations conduced, target group attendance, geographic location and program evaluation date in real time.
  • Ensures that the fidelity of evidence-based programming remains intact throughout the program implementation process.
  • Speaker and program evaluation data consistently meets or exceeds program expectations.
  • Assists in identifying and communicating needs for new or revision of existing programs and in-services to better meet the needs of the community.
  • Establishes positive, supportive relationships with participants and provide feedback.
  • Assesses participant needs for health education, to include change readiness.
  • Helps identify and implement participant education materials and other tools that encourage healthy decisions.
  • Helps participants develop health action plans and educate participants on self-care, healthful living, setting and achieving goals.
  • Follows up with health management/care plans with both participants and providers.
  • Coaches participants in effective self-management of their chronic / complex conditions.
  • Motivates participants to be active, engaged participants in their health.
  • Documents into electronic medical record the activities, health plans, and progress/outcomes in an effective manner while adhering to the policies and procedures of the Agency.
  • Assists patient in understanding care plans and instructions.
  • Assists patients in becoming independent to manage challenges and barriers.
  • Helps participants in utilizing appropriate resources, including scheduling appointments, transportation.
  • Continuously expands knowledge and understanding of community resources, services and programs provided; procedures and resources available to handle new, unusual or different situations.
  • Alerts Nurse Care Manager and or Supervisor on patient needs, concerns. Ensure continuity of care.

Requirements


  1. 3-5 years of experience in working with older adults
  2. B.S. in Health Education, Community Health or Public Health or related field

Expected Hours of Work

The employee will be scheduled to work Monday – Friday; 8 a.m. – 4:00 p.m.
