RN - Home Health Case Manager - FT Days - St. Peter Home Health

As a Home Health RN Case Manager with the Good Samaritan Society, you will play a vital role in coordinating and integrating healthcare services for our clients. This position involves working with a variety of patients, including admissions, discharges, recertifications, and routine visits. We are seeking a skilled and compassionate RN to join our team and provide high-quality care to our clients.
Responsibilities- Coordinate and sequence client care to ensure seamless transitions and optimal outcomes
- Develop and implement individualized care plans that address the unique needs of each client
- Collaborate with interdisciplinary teams to ensure comprehensive care and effective communication
- Monitor and evaluate client responses to care, making adjustments as needed to ensure optimal outcomes
- Document client information and care plans accurately and thoroughly
- Bachelor's degree in nursing preferred
- Minimum of one to two years of professional nursing experience in a clinic care setting
- Currently holds an unencumbered RN license with the State Board of Nursing
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
