Proactive, collaborative team member in a hospice care environment
Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
Commitment to caring for patients and partners
Complete all documentation and records in a timely and accurate manner
Follow established policies and procedures as related to the scope of practice
Maintain continuity of patient care in a hospice setting

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

Current, active, and unrestricted Registered Nurse (RN) and Nurse Practioner (NP) licensure in state of practice

Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care
