Title: Care Manager (RN)
Duration: 3 Months - potential FTE conversion
Location: Remote - Region 1 - Escambia, Okaloosa Santa Rosa or Walton Counties

Monday - Friday 8-5 EST
Day-to-day responsibilities:
  • Case management for children medical services (0-age 21)
  • Tier 4 & 3 members
  • Completing telephonic outreach monthly or quarterly
  • Home visits upon member enrollment, quarterly for tier 3 members.
  • Attending meetings, meetings for members, participation in regional meetings and activities that are required by the team.
  • All documentation is completed through TruCare, Teams, Zoom.
Performance expectations/metrics:
  • Caseload weighted at 200 (combined between tier 3 and 4 members)
Required Skills/Experience: Preferred Skills/ Experience: 1.Pediatric case management / pediatric nursing1.TruCare2. 2.Community Care3. 3.Medicaid experience (AHCA)Education Requirement:Requires a Degree from an Accredited School of Nursing or a Bachelor‘s degree in Nursing and 2 - 4 years of related experience.Education Preferred: Software Skills Required:TruCare, Teams, ZoomRequired Certifications:RN - Registered Nurse - State Licensure and/or Compact State Licensure requiredRequired Testing:n/aPosition is offered by a no fee agency.
