JOB SUMMARY::

A Licensed Practical Nurse (LPN) partnering with Radiant will work with Skilled Nursing facilities. This role is responsible for the daily clinical care needs of the residents.:

RESPONSIBILITIES:

* Provide primary nursing care to residents with varying backgrounds and medical histories
* Administer medications, wound care and other treatment options
* A Licensed Practical Nurse (LPN) will monitor and communicate changes of condition to provider
* Provide medical intervention as needed for emergent needs
* Communicate needs of resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc.
* As a Licensed Practical Nurse (LPN) you would communicate resident condition to family, and or responsible party members
* Implement and follow Primary Care Providers orders
* Other Duties as Assigned

Licensed Practical Nurse (LPN) Licensure in the State or practice:
