Trek Prosthetics is seeking a qualified individual to perform the necessary duties as a patient care coordinator to add to our growing team. The patient care coordinator plays a vital role in ensuring that our patients continue to receive the absolute best patient care experience.


Responsibilities

  • Answers incoming calls in a professional and courteous manner, promptly responding to requests and inquiries or directing/triaging calls for clinical personnel
  • Greets patients and visitors in a welcoming and helpful manner
  • Obtains patient demographic information and accurately enters and updates patient charts
  • Schedules patient appointments and ensures that all required follow-up is complete
  • Confirms future appointments following established guidelines
  • Performs pre-visit chart audit reviewing for required documents and plan of care requirements
  • Explains insurance requirements (PCP appointment, paper work, copay etc.) to patients and families and tracks to completion making follow-up outreach as needed
  • Collects documentation and patient payments as determined by insurance benefits
  • Documents all patient related activity in patient chart in an accurate, thorough, and timely manne



Qualifications

  • High School Diploma or GED
  • One - two years of medical office experience working directly with patients (preferred)
  • EMR/Nymbl experience (preferred)
  • Excellent communication and interpersonal skills with the ability to interface with patients, physicians, and coworkers in a tactful, informed and service-oriented manner
  • Ability to prioritize workload and manage required follow-up communications
  • Motivated to learn and grow within the company
