The position requires that you meet the CMS definition of a QIDP or QDDP to be considered.
Typical tasks include:Develop and implement individual service plans for a small caseload of intensive cases, including coordination and monitoring of care/servicesParticipate in the evaluation of the program’s quality assurance and compliance through quality reviewsProvide regularly scheduled supervision to assigned Clinician I and Clinician II employees to include training, assignment of cases, and evaluation workProvide program orientation to new staffComplete monthly probationary evaluations on new employeesComplete annual performance evaluations on assigned employeesParticipate and represent the agency in community outreachMaintain client records in accordance to program policies and procedures as well as state and federal regulationsReview and approve case management ISPs for assigned employeesPerforms other related duties as assigned. Requires a bachelor’s degree in social work, counseling, psychology, or a closely related field or another degree as approved by the Virginia Department of Health Professions.

SPECIAL CERTIFICATIONS AND LICENSES:
Requires a valid driver’s license with a driving record that is in compliance with City’s Driving Standards. Depending on assignment, may require Qualified Mental Health Professional (QMHP – Adult or Child) or Qualified Developmental Disability Professional (QDDP) certification and highest Part C standards within 6 months of hire; CPR, First Aid, and Medication Administration certifications within 3 months of hire; Certified Substance Abuse Counselor (CSAC) certification upon hire; Case Management Modules and Intellectual Disabilities Workbook completed within thirty days of hire; Part C Early Intervention Certification within six months of hire; and/or Pre-Screening Certification within 6 weeks of hire (12 weeks for part-time staff).

EXPERIENCE REQUIREMENT:
In addition to satisfying the vocational/educational standard, this class requires a minimum of three years of related, full-time equivalent experience.

SPECIAL REQUIREMENTS:
Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Preferred Qualifications In addition to the vocational/education requirements, this position requires 3.5 years of related, full time equivalent experience AND 1 year of supervisory experience.
