CLINICIAN II / Child and Adolescent Case Management
Are you an experienced and highly motivated Case Manager? Chesapeake Integrated Behavioral Healthcare is seeking a case manager who enjoys working with children and families to support their wellness journey.
The Clinician II will perform the below duties:
  • Assess support needs and develops individual service plans and implement plans in compliance with State Licensure and Medicaid regulations and standards.
  • Assist individuals by linking, locating, monitoring, developing needed services, resources, benefits and making collateral contact with the individuals' significant others to promote implementation of the individual service plan.
  • Coordinate services with other agencies and providers involved with the individual, maintain necessary records to document the provision of case management services. If the individual's needs require, utilize crisis services and medical services.
  • Clinician II will monitor service delivery, make home and other agency site visits to observe and evaluate individuals in their home and community environment.
  • Supports individuals with accessing FAPT services to include completing FAPT application, presenting cases to FAPT team, link to FAPT funded services, monitor FAPT funded services, complete monthly FAPT reports.
  • Clinician II will accept initial referrals, complete intake evaluations, assess client needs, determine service needs, communicate effectively with individuals, families and significant others.
  • Performs other related duties as assigned

Required Qualifications
VOCATIONAL / EDUCATIONAL REQUIREMENT :
Candidates must have BS in Human Services (psychology, social work, special education, etc.) preferably supplemented by considerable experience in working with persons who experience intellectual/developmental disabilities.A master's degree in Social Work, Counseling, or Rehabilitation is required for some assigned programs.
Bachelors degree in human services field or other degree as approved by the Virginia Department of Health Professions is required for positions requiring QMHP -C certification

EXPERIENCE REQUIREMENT :
In addition to satisfying the vocational/education standards, this position requires a minimum of two years of Child and Adolescent case management full-time equivalent experience.

SPECIAL CERTIFICATIONS AND LICENSES :
Requires a valid driver's license and a driving record that is in compliance with City Driving Standards .Must have QMHP -C certification - Cannot accept QMHP -Trainee
Prior experience with C & A Case Management Preferred.

SPECIAL REQUIREMENT :
Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.
