The Position

Incumbent works under the direction of the Director of Psychiatric Nursing or designee, coordinating mental health care of clients throughout the treatment process from initial assessment to discharge planning.


Essential Functions

  • Conduct bio-psycho-social assessments for new patients to determine level of risk, coordinate inpatient psychiatric referrals, and discharge planning.
  • Establish and maintain effective work relationships with clients, supervisors, County employees, elected officials, attorneys, law enforcement, judges, other agencies, and the public.
  • Comply with Civil Service policies and regulations, collective bargaining agreements, County policies, department policies and laws to create a cooperative, safe, respectful and quality work environment.
  • Round daily with staff sharing information on patients‘ condition and issues.
  • Participate and develop treatment plans based on initial assessments and interviews in collaboration with an interdisciplinary treatment team.
  • Implement treatment plans serving as a patient advocate throughout the process.
  • Assist in placement of appropriate care level (e.g. group, individual, substance abuse) for patients based on individual needs.
  • Work with appropriate staff to modify treatment plans based on changes in patients‘ status.
  • Coordinate discharge planning with family members, acquaintances and outside agencies.
  • Consult with staff regarding patient needs and available support services.
  • Develop therapeutic relationships, maintaining a professional attitude toward clients and treating them with empathy and respect.
  • Perform individual and group therapy.
  • Provide substance abuse evaluation and counseling.
  • Collaborate with CMHC staff.
  • Utilize applicable state and federal healthcare rules and regulations.
  • Maintain knowledge of local and state community agencies assisting with mental health issues/services; assist family and patients with community-based services.
  • Analyze data, identify trends and report concern.
  • Maintain knowledge and job skills (e.g. research, webinars, seminars, conferences).
  • Safely operate a motor vehicle when required to travel on County business.
  • Report to work with regular, consistent attendance.
  • Perform other duties as directed and assigned.

Education and Work Experience and Other Requirements

  • Master‘s degree from an accredited college or university in Social Work, Psychology, Sociology, or other directly related field of study required.* Social Work preferred.
  • Completion of practicum in mental health setting required.
  • Two (2) years of experience in the human or social services field required.*
  • Certified or provisional Licensed Mental Health Professional (LMHP) required.
  • Completion of Cardiopulmonary resuscitation (CPR) and Crisis Prevention Intervention (CPI) training within 6 months of hire required.
  • Valid driver‘s license upon hire and maintained throughout employment required.
  • Completion of a pre-employment criminal record check and conditional offer drug screen required.
  • *Equivalent combination of education and work experience may be substituted for requirements on a year-for-year basis.

Physical Requirements & Working Conditions

  • The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.   • Work is performed indoors in a locked psychiatric facility with mental health patients who can be a danger to themselves and others. Noise level is usually moderate but could be extreme. Work hours are typically standard day-shift hours; however, schedule may vary (e.g. weekends, holidays, overtime/extra hours, emergency call-in). Work involves potential exposure to biohazards and risk of injury from assaultive/abusive patients. Work may be stressful when dealing with time constraints, multiple/changing priorities, limited resources and uncooperative/uncontrolled individuals.
  • Work requires physical activity including extended periods of standing, walking, frequent sitting, bending, reaching occasional kneeling, climbing, stooping, crouching, and balancing. Work also requires the ability to frequently lift/carry objects weighing up to 50 pounds and occasionally up to 75 pounds. Additionally, incumbents must be able to exert 65 pounds of grip strength while quickly changing position from kneeling to standing.
  • Required sensory abilities include vision, hearing and touch. Visual abilities, correctable to normal ranges include close, distance, peripheral, and color vision, depth perception and the ability to adjust focus. Communication abilities include the ability to talk and hear within normal ranges. Incumbent must possess the hand-eye coordination and manual dexterity necessary to operate computers and other equipment.
