Summary:

The Integrated Behavioral Health clinician III is an independently licensed LCSW embedded within an outpatient medical setting as a psychotherapist.

They directly with the primary care team to assess and treat individuals with behavioral health and/or substance use issues (either on-site or via telemedicine) .

They understand or are willing to learn about the collaborative model of care applied in a patient-centered medical home.

They adapt interventions to the workflow and pace of the primary care practice, including rapid assessment, brief treatment, and a high daily volume of patient contacts.

They frequently consult with primary care providers in real time to assist patients with psychosocial issues and utilize "warm hand offs" during hours of clinical operation to enhance patient engagement with the primary care team.

They are a clinical leader, a consultant, and a resource at the medical practice in program development, actively participating in group decision-making, implementing plans, and improving quality of care.

They are an enthusiastic learner who actively seeks research and knowledge to ensure current, evidence-based practice while adhering to key components of collaborative care.

This clinician performs duties and/or responsibilities within their scope of practice as assigned by the Clinical Supervisor of Behavioral Health Integration.
