Assistant Chief Nurse, Primary CareThe Assistant Chief Nurse, Primary Care (ACN/PC) is responsible for clinical and administrative activities of Primary Care units to include the Community Based Outpatient Clinics (CBOC), Home Tele-Health and Telephone Advice Program (TAP) in collaboration with the CN,PC.
S/he executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines, influence organizational mission and health care, and responsibility for outcomes.
The ACN,PC is collaboratively responsible for administrative and operational functions of his/her assigned outpatient care areas and functions as an administrator, educator, and consultant utilizing management theory in collaboration with Nursing and Patient Care, Medicine, Surgery and all other services supporting care in the outpatient care areas.
The ACN/PC in collaboration with primary care leadership will be a major contributor for the implementation and evaluation of major primary care initiatives, i.e.
Patient Aligned Care Teams (PACT).S/he must establish and maintain effective relationships with all levels of medical center personnel and relate effectively with customers, families, and the community as well as program and service leaders at the local and network levels.
