HBPC is a home care program under the Geriatric and Extended Care Service Line, designed to meet the longitudinal, primary care needs of an aging Veteran population with complex, chronic, disabling disease.

HBPC provides a patient centered approach to care and treatment in the patient's home. The HBPC RN is a foundational member of the interdisciplinary care team for reasonably home bound, chronically ill Veterans.

The HBPC RN functions as a care manager for the HBPC assigned Veterans and is responsible for continuous assessment of the Veteran and caregiver needs; delivering nursing care in the home; and supervising the care of home health technicians if applicable.


Included but are not limited to:
Accurately performs assessments/screens for nutrition, pain, tobacco use, alcohol, spirituality, depression and SLUMS
Coordinates outcome focused interdisciplinary care plans that reflect the team's assessments, interventions, evaluations and follow-up
Implements appropriate nursing and team plans of care
Demonstrates ability to care manage HBPC clients through the timely completion of screening tools and referrals to other services
Provides assessments and documentation of HBPC patients
Administers medications following established Medical Center and Service Line policies and procedures
Assesses patients for abnormal or non-therapeutic reactions to medications and initiates appropriate interventions
Demonstrates ability to perform wound care according to treatment orders
Educates caregiver in techniques for turning and positioning of patients
Demonstrates ability to train others in suctioning/care for NG tube
Demonstrates ability to integrate telehealth technology into practice
Applies evidence-based standards to screen, immunize and promote health activities in the patient
Demonstrates the ability to perform phlebotomy in the home and transport blood draws to the Lab
Maintains government vehicle and mileage reports
Accurately completes required workload reports

Pay:
Competitive salary, regular salary increases, potential for performance awards

50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
