DUTIES:
Performs the following functions in addition to those listed in (a) Prescription Processing.
(1) Provides additional patient specific profile review to include:
(a) Therapeutic appropriateness of a patient's drug regimen
(b) Degree of patient compliance with the prescribed drug regimen.
(2) Provides medication counseling to patients and/or caregivers
(3) Completes medication history including patient interview
(4) Performs medication reconciliation updating the medication profile to reflect an accurate, active list of VA and non-VA medications. This may include adding non VA medications or discontinuing duplicate medications or those the patient is not taking.
(5) Communicates with Providers to coordinate care and delivery of medications in a timely manner to prevent therapy interruption.
Prescription Processing
(1) Provides patient-specific profile review and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing.
Profile review will include prospective review and intervention in:
(a) Therapeutic duplication in the patient's drug regimen
(b) Appropriateness of the drug, dose, frequency, route of administration and patient instructions for use
(c) Drug - Drug interactions
(d) Allergies and adverse reactions
(e) Potential abuse of medication
(f) Potential errors with look-alike and/or sound-alike drug pairs
(2) Refers medication-related problems to MbM Resolution Team
(3) Reports adverse drug events (ADE), near misses and medication errors in alignment with the Meds by Mail reporting program
(4) Enters supply orders appropriate for patient care per MbM policy
(5) Manages various actions, as appropriate for medication orders to include flagging prescriptions, and using the 'hold medication' function
