Job Summary

The Practice Transformation Project Manager II in support of specified projects and/or initiatives, plays a key delivery role in helping physicians transform their practice and improve clinical outcomes and patient care. This position will work independently in carrying out the goals of the program/initiative(s), and collaboratively with practices, program principals and the Practice Transformation team. This position uses their knowledge to assist practices to achieve the program/initiatives' goals and improve clinical outcomes in a fluid program environment.

Duties

Work together with the practice(s) to develop and implement the practice work plan in support of the program/projects and/or initiative's goals designed to achieve improvements in health care delivery and health outcomes for patients. Provide feedback to help update relevant areas of the work plan using Practice Transformation program management tools and manage issue resolution effectively.

Provide technical assistance to practices which will be tailored to meet practice needs and may include supporting provider outreach and education, practice readiness, practice facilitation, health information technology ("HIT") optimization, practice workflow redesign, change management and strategic planning. Technical assistance may be provided on-site and remotely using a set of work plans and interventions to support the program/project and/or initiatives as defined by L.A. Care and the practices to improve quality measures and transform their delivery of care.

Track the project goals, clinical quality measures and other program data and objectives in the practices' electronic health records ("EHR") system, and report data results monthly to the Practice Transformation team, as required. Participate in coach workgroups and other learning sessions to share learnings and gain knowledge to better provide facilitation/teaching and technical assistance.

Conduct regular practice assessments using L.A. Care program tool(s)/approaches.

Work with L.A. Care, program/initiatives leads, and practices to implemented project strategy, help with intervention design, and ensure deliverables are completed on time, within budget, and with high quality.

Bachelor's Degree in Health Administration or Healthcare Related FieldIn lieu of degree, equivalent education and/or experience may be considered.Education Preferred

Required:

At least 3-5 years of experience in the health care industry with implementing Plan-Do-Study-Act ("PDSA") or similar quality improvement methodologies in a primary care, behavioral health or human services setting.

Experience with Electronic Health Record software implementations and meaningful use experience and/or experience in quality improvement programs and work flow processes in clinical settings.

Experience in working at medical practices with providers and staff/care teams.

Skills

Required:

Knowledge of workflow process in clinics/practices and quality improvement reporting.

Possess strong interpersonal skills in the healthcare arena.

Must have implemented a Plan-Do-Study-Act ("PDSA") or similar quality improvement methodologies using a data driven methodology.

Strong project management, communication, relationship management skills and health I.T.

Excellent communication skills.

Able to work independently and regularly demonstrate initiative.

Able to work effectively in a health care team environment and manage issue resolution effectively.

Ability to work and resolve interpersonal conflict.
