Department Overview
We believe our well coordinated clinical care contributes to our outcomes that are among the best in the country.
Work as part of team that includes Cancer and Blood Disorder physicians, nurse practitioners, physician assistants, nursing, social work, physical therapy, nutrition, child life, clinic assistants and researchers.
The department at Children's of Minnesota provides care for children of all ages with all types of malignancies including brain tumors, solid tumors, and leukemia; and blood disorders such as sickle cell disease, bleeding disorders, and anemia.
As the largest provider of services for children with cancer and bleeding disorders in the region, the Cancer and Blood Disorders program focuses on providing well-coordinated knowledge-driven clinical care.
We participate in national and international research and are leaders in research on supportive care and quality of life for children with cancer and bleeding disorders.
Position Summary
The Sickle Cell Patient Health Advocate works in collaboration and under the direction of the clinical team to support patients and families with Sickle Cell Disease (SCD) navigate the health care system and community resources ensuring engagement and access to care.
Collaborates with the health care team to develop and implement culturally appropriate programing and health care information to support patients and families and raise awareness about SCD.
Education:
*Bachelor's degree or equivalent required
*Community Health Certificate preferred
Required Experience:
*Demonstrated experience working with family systems
*Two (2) years related work experience
*Experience working with diverse communities
Knowledge/Skills/Abilities:
*Able to manage multiple projects and responsibilities at once, ability to meet deadlines
*Ability to perform under stress and thrive in a fast-paced intense work environment
*Excellent written and verbal communication skills
*Positive attitude and ability to work well in a team
*Cultural mediation skills
The posted pay range represents rates for a grouping of multiple jobs within a salary grade.
We carefully consider a wide range of factors including but not limited to market indicators for the specific role, the skills, education, training, credentials and experience of the candidate, and organizational needs.
Base pay is just one piece of the total rewards program offered by Children's Minnesota.
All job offers are contingent upon successful completion of an occupational health assessment, drug screen, background investigation, and compliance with the U.S.
Children's Minnesota requires proof of COVID-19 vaccine, and as recommended by the Centers for Disease Control and Prevention (CDC), immunity to vaccine preventable infectious diseases, prior to employment.
