  • Participating in patient education and care coordination efforts;
  • Monitoring and evaluating treatment plans for chronic care patients;
  • Collaborating with the healthcare team and patients to achieve healthcare goals;
  • Facilitating patient self-management through education and communication;
  • Overseeing the development and adoption of self-management educational resources;
  • Developing and maintaining relationships with external healthcare resources;
  • Ensuring the successful implementation of treatment care plans and continuity of care.
The ideal candidate should be 18 years or older, have RN licensure, and preferably an RN, BSN. Professional requirements include CPR, ACLS, and PALS certifications, along with experience in clinical settings and care coordination. Being bilingual is a plus. This full-time position may require weekends and travel, offering a competitive salary and benefits package. Job Type: Full-time Schedule:
  • Monday to Friday
  • Weekends as needed
