Aspirus Wausau Hospital in Wausau, WI is seeking a Team Lead of Social Work- Care Coordination to join our team

Team Lead of Social Work assists patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation. Works with patients and families to obtain psychosocial history, assist patients in resolving environmental difficulties interfering with attainment of maximum benefits from medical care, and conduct routine discharge planning. Directs patients to designated community agencies or resources, as necessary. Works with clinical social work staff to foster collaborative practice that will optimize use of resources and assure customer satisfaction. Accountable for evaluations, supervision, leadership, and fiscal outcomes of the Social Work team members.

HOURS: 1.0 FTE/80 hours every two weeks, days; weekends and holidays based on rotation

Experience/Qualifications
  • Master's Degree in Social Work from a social work curriculum accredited or approved by the Council on Social Work Education.
  • Coursework in Medical Social Work preferred.
  • Approximately six months to a year of on the job experience necessary in order to gain full knowledge of federal, state, and local regulations related to discharge planning and knowledge of health and welfare resources and facilities.
  • Leadership experience is a plus
  • Possesses Wisconsin Certified Social Worker licensure.

Aspirus Wausau Hospital is the flagship of the Aspirus system that serves patients in 14 counties across northern and central Wisconsin, as well as the Upper Peninsula of Michigan. It is a licensed private/not-for-profit facility with 325 beds and staffed by 350 physicians in 35 specialties.
