Job Description:
  • Participates in community events and activities promoting health maintenance and disease prevention
  • Conducts home visits to determine client needs and assist in care plan development and implementation
  • Assists clients in accessing community resources and arranges medical appointments
  • Refers clients with challenges to appropriate service agencies
  • Collaborates with clients, families, and other professionals to coordinate services and provide education on wellness and disease prevention
  • Documents services provided and contributes to program development and evaluation
  • Coordinates group meetings and collects necessary evaluation data


Qualifications:

  • High School Diploma and completion of the CHW Training Program (within 180 days of hire)
  • Associate's Degree preferred
  • 3 years of relevant outreach work experience preferred
  • Familiarity with community resources and social services
  • Basic Life Support certification required within specific timelines
  • Driver's License in the State of Michigan required

Weekly Scheduled Hours: 40
