Position Overview:
As a Biopsychosocial Care Provider, you will be responsible for conducting assessments, offering crisis intervention, providing short term counseling, advocating for patients and their families, and ensuring smooth transitions of care in various patient settings.
You will also deliver psycho-education and may lead support groups and educate staff on psychosocial aspects of care.

Below are the main responsibilities, requirements, and qualifications for this role:
JOB ACCOUNTABILITIES:
- Conduct thorough biopsychosocial assessments and develop care plans for patients and families.
- Provide direct clinical services such as discharge planning, individual and family counseling, crisis intervention, and linking patients with resources.
- Advocate for patients and families to ensure access to care and protect their safety and rights.
- Document all interactions accurately and promptly in patient records and hospital records.
- Establish and maintain positive relationships with hospital departments, medical staff, and community representatives.
- Promote mutual support and professional excellence within the department.
EDUCATION:
Master's: Social Work
TYPICAL EXPERIENCE:
Minimum of 2 years of recent relevant experience
SKILLS AND KNOWLEDGE:
Skills in conducting biopsychosocial assessments, crisis intervention, grief counseling, and more.
Collaboration skills and ability to work effectively in a team.
Knowledge of transitions of care, community resources, and clinical social work regulations.
Understanding of mental health conditions, substance use, cultural influences, and end-of-life care.
Strong communication skills, both verbally and in writing.
Proficiency in using computer software programs for data entry and communication.
Familiarity with local resources for at-risk populations.
