Key Responsibilities- Conduct thorough biopsychosocial assessments to identify patient and family needs, strengths, and concerns.
- Develop and implement plans of care to address patient and family psychosocial needs, including crisis intervention and short-term counseling.
- Collaborate with healthcare teams to provide education and support to patients and families, promoting effective coping mechanisms and behaviors.
- Link patients and families to community resources and services, facilitating transitions of care and promoting patient safety and well-being.
- Advocate for patients and families, promoting their rights and interests in a culturally sensitive and compassionate manner.
- Master's degree in Social Work (MSW) from an accredited program.
- 2 years of recent experience in medical social work, with a focus on biopsychosocial assessment and intervention.
- Strong clinical skills, including crisis intervention, counseling, and case management.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Knowledge of local community resources and services, with the ability to link patients and families to appropriate support.
