JOB SUMMARY: Responsible for providing quality primary health care to patients while working closely with other healthcare staff in a team approach.

KNOWLEDGE OF WORK
Knowledge of Electronic Health Records and related computer systemsAbility to collaborate with mutual providers of treatment in mental health and substance use settings as well as work well with the primary care teamAbility to prioritize and multi-task and have strong time management skills
DUTIES AND RESPONSIBILITIES
Provide the following health care services for patients: assessment/evaluation, diagnosis, treatment/prescribing, planning and education. Consult with staff physicians as neededReview and manage patient information including labs, diagnostic images, prescription refills, outside consultation notes, previous medical records, specialist referrals, etc.Prescribe and administer medications and treatment regimens based on assessed medical conditions. Review medication(s) that patient is currently taking, prior to prescribing to prevent adverse reactionsRefer patients to specialists and to relevant patient care services as appropriatePrioritize patient care throughout the dayProvide acute triage over the phone or with walk in patientsProvide education and information to patients on various issues, providing handouts as appropriatePerform various in office procedures as directedDocument all appropriate information in electronic health record systemRespond to messages and relay information to patients and any other internal or external sources as neededComplete various medical related forms as neededDirect and coordinate the patient care activities of nursing and support staff as appropriateResearch on unusual casesMay participate in and collaborate with staff/teams on various support-related functions including quality assurance, care team development, system optimization, empanelment, policy and procedure development, etc.Assists in the coordination and integration of all health clinic outreach programs as directedParticipate in education of studentsPerform other duties as assigned

EDUCATION, EXPERIENCE, TRAINING FOR PA

  5. Must have or obtain current DEA License
  6. Current Basic Life Support CPR certification required
  7. Experience in a community healthcare center preferred
