The Lead Vocational Rehabilitation Specialist plays a crucial role as part of the Program for Assertive Community Treatment (PACT) multidisciplinary team focusing on vocational and employment areas.

This specialist delivers treatment, rehabilitation, and support services while also leading employment services.

  • Manage case files and provide ongoing assessment for an assigned group of individuals, overseeing the activities of the individual treatment team (ITT)
  • Take the lead in creating, implementing, evaluating, and revising Individual Action Plans (IAP) in collaboration with ITT
  • Offer individual supportive therapy and symptom management, adjusting the IAP as needed; educate and support individuals and their families, and advocate for their rights and preferences.
  • Perform a thorough assessment covering psychiatric history, mental status, physical and dental health, substance use, education, employment, social development, daily activities, and family relationships
  • Lead or participate in providing rehabilitation services
  • Provide vocational-supportive counseling to help individuals identify strengths, establish goals, address mental health challenges affecting work, and develop job-seeking skills
  • Conduct on-the-job assessments, work reviews, job coaching, problem-solving, and support both on and off the job site
  • Carry out job performance evaluations, work closely with employers, and provide crisis assistance
  • Fulfill other duties as necessary
