- Develop and maintain relationships with community organizations and healthcare providers
- Bachelor's Degree in a related field
- 3-5 years of experience in hospital discharge planning, long-term care, or community health
- Strong knowledge of regulations, community organizations, and healthcare systems
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
