About the Position:

  • The Reaching Out on the Streets (ROOTS) Program focuses on reducing homelessness by addressing social determinants of health, empowering individuals, and intervening in critical moments to enhance access to services and decrease fatal overdoses.
  • The ROOTS Senior Outreach Specialist is responsible for community-based outreach and support for individuals facing housing instability or homelessness within various city areas.

Responsibilities:

  • Conduct community outreach for individuals experiencing homelessness.
  • Advocate for clients facing service barriers.
  • Support transition from housing instability to secure housing.
  • Establish relationships with shelters and external providers.
  • Distribute harm reduction supplies.
  • Assist in obtaining housing documentation.
  • Provide crisis intervention and risk assessments.
  • Document service encounters and outcomes.
  • Support multiple sites across Pittsburgh.

Requirements:

  • Three years of experience in related fields.
  • High school diploma required, bachelor's degree preferred.
  • Educational training may substitute work experience.
