About the Opportunity:

  • Start Date: ASAP
  • Assignment Length: Ongoing (Contract to Hire)
  • Schedule: Monday to Friday
  • Hours: 8am to 5pm or 9am to 6pm
  • Setting: FQHC
  • Patient Population: Young Adults & Adults
  • Patient Volume: 10 to 20 per day
Responsibilities:

The Substance Use Counselor will:
  • Assess patients' behavioral health needs
  • Provide substance use prevention education
  • Conduct screenings and brief interventions during and/or after medical appointments
  • Provide individual and group counseling
  • Refer patients with moderate to severe behavioral health needs to onsite Behavioral Health team or community mental health institutions
  • Refer patients with moderate to severe substance use to the onsite Medication Assisted Treatment program or to community partners specializing in detox, intensive outpatient and residential treatment
  • Perform other related work, as assigned
Qualifications:
  • 2+ years of experience working with Adults with Dually Diagnosis and/or Substance Use issues
  • Current Licensed Chemical Dependency Counselor in good standing
  • Demonstrated significant knowledge of Behavioral and Primary Integrated Care models
  • Basic knowledge of Cognitive Behavioral Therapy based counseling methods and techniques
  • Basic knowledge of the Counseling 12 Core Functions of drug and alcohol substance use/abuse
  • Basic knowledge of 12-Steps programs and other support groups
  • Basic knowledge of case management
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • Graduate from an accredited college or university with a Bachelor's and/or Master's Degree in Counseling, Social Work, Behavioral Science or a closely related field
  • Experience working in a Medical setting
  • Experience working with Electronic Health Records
