Job Description

Purpose of Position:

The Supervising Medical Records Specialist under the supervision of the Director and/or Manager of the Health Information Management Department is responsible for competently, accurately, and correctly assigning ICD-10CM diagnostic, procedural codes, CPT, and HCPCS codes for reimbursement, educational, and statistical information.

Examples of Typical Tasks:
1. Thoroughly reads and interprets the documentation contained in every medical record to identify all diagnoses and procedures to which codes must be affixed.
2. Performs coding for Inpatient, ER, and Ambulatory Care records.
3. Input coded information into the Epic/3M encoding system
4. Applies Uniform Hospital Discharge Data Set definitions to select the principal diagnosis, principal procedure, and other diagnoses and procedures, that require coding. Understanding coding guidelines
5. Select the appropriate principal diagnosis for episodes of care in which the determination of the principal diagnosis is not clear-cut because the patient has multiple problems.
6. Applies sequencing guidelines so the chronology of patient care events can be understood by reviewing the assigned codes.
7. Understand the DRG and Case-mix Methodology
8. Assesses and collaborates with CDIS in promoting adequacy of documentation to support the assigned codes for principal diagnosis, principal procedure, complications, and comorbidity conditions.
9. Partner with CDIS in querying providers to verify clinical indicators, diagnosis specificity, details of the procedures, and clarification of conflicting ambiguous documentation found in the medical record

a. Adhere to corporate and facility HIM workflow to expedite completion of work

b. Assist in work queue management, medical record reviews and validation, chart
10. Analysis, clinical, and other HIM functions.
11. Maintains LMMHC &HIM Safety Program, Quality Improvement Program, and In-Service Program.
12. Supervise Associate Medical Record Specialists and other HIM staff
13. Train medical record personnel and other administrative staff

Assist with reporting and other related functions within the scope of the Supervising Medical Records Specialist

Minimum Qualifications
1. Bachelor's Degree in Medical Record Administration with at least one year experience as a Medical Record Specialist or Medical Record Technician.
2. Completion of a course of study for Medical Record Technicians approved by the American Medical Record Association and two years experience as a Senior Medical Record Specialist or Medical Record Technician, one of which must have been in a supervisory capacity; or,
3. A certificate issued upon completion of the Correspondence Course for medical record personnel given by the American Medical Record Association plus the experience listed in item (2); or,
4. High School graduation or evidence of having passed an examination for the High School Equivalency Diploma or the U.S. Armed Forces GED certificate with a score of at least thirty-five on each of the five tests and an overall score of at least two-hundred-and-twenty-five in the examination for the certificate and three years experience as a Senior Medical Record Specialist or Medical Record Technician, one of which must have been in a supervisory capacity; or,
5. A satisfactory equivalent of education and experience.
