A Housing Care Manager contributes to AHF by assisting clients with access to resources, working with potential and existing tenants to remove barriers by facilitating a collaborative process that identifies clients needs, a plan to address those needs, and continuous follow-up to ensure that members obtain the necessary services that help them to maintain successful residency. The HCM coordinates with other service providers through a comprehensive and active referral process to link appropriate services and care to clients.

A Housing Care Manager enhances the reputation of AHF by utilizing your strong skill set and a caring and mission-driven focus, you will work with AHF clients, external agencies, area housing coalitions, and other related resources to implement, support, and monitor the housing case management needs of the clients. You will serve as a key component in improving the residents stable housing options focused on successful residency and improved quality of life

The Housing Care Manager will work as a part of a close-knit team to develop a comprehensive database of resources designed to address client needs, including initiatives that incorporate wrap-around services that are equitable, inclusive, and tailored to the needs of individuals and families. The HCM coordinates with other service providers through a comprehensive and active referral process to link clients to appropriate services and care.
