Position Summary:
- The Inpatient Care Manager collaborates with the clinical team and medical provider to coordinate discharge planning and interventions aiming to promote patient wellness, enhance care outcomes, utilize health services efficiently, and reduce payment denials among patients with complex health needs.
Requirements:
- Basic professional knowledge; equivalent to a Bachelor's degree; working knowledge within a specialized field
- BSN preferred; ADN required
- 3-5 years of related experience
