Position Summary:
Provide comprehensive care to individuals to help them achieve autonomy and enhance productivity.

  • Responsible for planning, developing, and implementing goals and training to enhance the skills of individuals served in various areas such as budgeting, grooming, nutritional planning, use of public transportation, community resources utilization, shopping, housekeeping, physical health, adaptive behavior, independent living, nutrition, vocational and social development.
  • Monitor the progress of individuals served.
  • Provide training in daily living skills.
  • Coordinate emergency evacuations and ensure resident safety during emergencies like fire drills.
  • Supervise medication self-administration by individuals served.

Abilities required include:
Reading, writing, communication, observation for side effects, and monitoring medical conditions like blood sugar levels.

  • Responsible for crisis intervention and incident reporting.
  • Monitor and document progress on case notes, collateral notes, progress notes, and house log.
  • Assist in planning leisure activities and participate in physical activities.
  • Attend staff meetings, training sessions, and meetings within and outside the residence.

Must complete training courses such as Medication Supervision, CPR and First Aid, Cultural Competency, Mandated Reporting, Corporate Compliance.

  • Act as a role model for individuals served.
  • Maintain confidentiality of records.
  • Assist with lunch preparation, clean up, and clothing care.

Active participation in program planning team activities, ability to read and write various documents, lift/carry supplies are essential.

Other Responsibilities:
Follow-up on treatment recommendations, accompany individuals to appointments, assist in accessing community resources.

Ability to adapt to changing work schedules and locations. Maintain confidentiality at all times.

Qualifications: High School Diploma or equivalent required. Experience with adults with behavioral health or disabilities preferred.
