The Senior Patient Relations Specialist serves as a proactive patient advocate in helping patients "navigate" through the health care system.

Will serve as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within St John's Riverside Hospital and in the community.

Facilitates patient appointments, including those made with labs, diagnostic areas and specialty physicians. In addition, will lead other Patient Relations Specialists in training and workflow.

Provide individualized assistance to patients to facilitate access to quality care. Organizes, schedules, and manages appointments for patients to ensure they receive post-discharge services in a timely manner. Facilitates appointments for diagnostic services as needed. Provides the patients with information for medication assistance programs and meds-to -beds program available to them via Sunlight Pharmacy.
Provides assistance for a smooth transition as the patient leaves the hospital. Ensures continuation of care and linkage to community resources. Follow up with patients post-discharge who were identified as needing additional support with their post discharge care.

Improves communication between the patient, family members, and healthcare providers to ensure patient satisfaction. Accommodating interpretive language services when needed.

Builds and maintains a relationship with medical staff, nursing, pharmacy, case management, social services, community liaisons, community providers and mental health facilities.

Collects and summarizes data from internal systems to demonstrate areas of progress and areas of improvement. Will lead in training and designing a workflow for other Patient Relations Specialists.

  • Undergraduate Degree or Pending required.
  • Demonstrates ability to communicate orally and in writing, using advanced writing skills.
  • Demonstrates proficiency in MS office suite of products.
  • Proficient in the use of hospital health information system.
  • Requires organization, problem solving, decision making and critical thinking.
  • Must be able to work in a self-directed environment
