The rehab case manager is responsible for the coordination of services required for an organized multidisciplinary team approach to patient care.

These duties are to be performed in a highly confidential manner, in accordance with the mission, values and behaviors of Insight Hospital and Medical Center.
Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers and guests.
Responsible for the coordination of services required for an organized multidisciplinary team approach to patient careAssures quality cost-efficient care for the rehab patient population
Works collaboratively with interdisciplinary teams, department members and other healthcare provides both internal and external to the organization to improve patient care through effective utilization and monitoring of healthcare resources
Assist patient and/or designee in making appropriate referrals depending on patient level of care/needs in the Community
Responsible for therapeutic treatment plans on assigned patients and assisting in process when necessary/requested
Maintains necessary Case Management documentation based on referral source's needs and as applicable on a case-by-case basis
Monitors post discharge services as applicable and provide needed referrals
Participates actively in team and department meetings
Serves on hospital committees as designated
Qualifications:
MSW degree with LSW required
Excellent written and verbal communication skills
Licensure to practice in the State of Illinois
Two (2) years experienced preferred, preferably in rehabilitation setting and/or discharge planning.
