Job Summary


The Manager of Population Health and Wellness is primarily responsible for the population health needs across all lines of business and coordinate the delivery and evaluation of population health and health promotion programs, campaigns, and other targeted member outreach to promote healthier lifestyles and better health outcomes.

Outreach may include but is not limited to, member communication about gaps in healthcare needs and preventive health opportunities, member and provider education, communication campaigns, and medical management referrals.

Responsible for regular reporting, data analysis and evaluation of targeted member outreach efforts.

This individual will play a key role in developing and deploying the infrastructure for data utilization in order to achieve the program goals.


Minimum Qualifications

Education:
Bachelors degree required; Masters degree preferred.

Experience / Knowledge / Skills:

  • Five (5) years minimum experience in a customer service environment with specialization in education, population health management programs, quality assurance monitoring, and/or training.
  • Understanding of health care data and valuebased contracts.
  • Proven experience in developing, implementing, monitoring and measuring population health management programs.
  • Strong research, problemsolving, and analytical skills.
  • Ability to effectively utilize and leverage technology systems and understand data analytics.
  • Ability to effectively collaborate with multidisciplinary stakeholders.
  • Skilled in critical thinking & resultsoriented process improvement.
  • Excellent coordination skills, including multitasking and setting priorities on work assignments.
  • Excel skills required.
  • Demonstrated program or project management.
  • Experience in working in a team environment that promotes coaching and training and positive feedback to improve individual performance and customer satisfaction required.

Principal Accountabilities

  • Analyzes employers overall health risk factors and provides detailed reports, designed to determine educational opportunities and environmental changes.
  • Develops, manages, and implements business plans to determine the needs of the client and generate new business.
  • Establishes relationships to determine business needs.
  • Develops and implements initiatives based on assessment to meet the needs of the community.
  • Advises employers on establishing a healthy workforce and reduction of healthcare costs by identifying, improving and reducing health risks.
  • Develops and maintains positive public relations between internal and external customers and vendors and serves as a resource to members, employees, employers, and the community.
  • Collaborates with technical and analytical resources to deliver complex solutions to realworld, adhoc data analysis and custom reports.
  • Designs and modifies existing dashboards and reports and other content as appropriate, and work collaboratively with colleagues on configurations and modifications that would optimize the client experience.
  • Provide guidance to the client's management team in development of best practices documentation and content modification/creation of report and dashboard templates Work with the product innovation staff to design, test and implement new reports and wellness portal customizations and functionality.
  • Communicate product and services updates to clients and coordinate training as appropriate.
  • Manage and execute developed plan and timeline for key deliverables.
  • Manages and oversees Comprehensive Health and Wellness services, including but not limited to: seasonal vaccinations, biometric screening, wellness portal, client incentive programs, and health coaching and disease management.
  • Organizes, staffs and deploys onsite screenings, events, etc. based on the employers needs that are scalable to each client managing multiple events and multiple groups of contractors may be utilized at any given time and on the same day.
